New Patient Information

Your First Visit

We want to be sure your teeth and gums are healthy, and prevention is very important in achieving this goal. When you visit our office for the first time, we’ll take the necessary X-rays, do a tooth-by-tooth inspection, assess your gums for any signs of disease, screen for oral cancer, and examine your bite. You’ll also receive a thorough professional cleaning with one of our skilled hygienists. If we spot any signs of trouble, we’ll discuss it with you.  If you have any questions or concerns, be sure to mention them to Dr. Lee or a member of the team. We’ll be sure you leave our office feeling confident and informed.

New Patient Forms

As a courtesy and convenience, we’ve provided our new patient forms online. Feel free to print them off at home, fill them out when you have time, and then drop them off with our front desk when you check in. If you prefer to fill out your paperwork in the office, be sure to arrive about 15 minutes early.

Insurance & Payment Options

Lee Dental Group accepts most insurance companies, including Delta Insurance plans, and our front desk is happy to help you understand your benefits and file paperwork on your behalf. We accept cash, check, and major credit cards (Visa, MasterCard, and Discover) as forms of payment.

Financing Available

Our office has also partnered with CareCredit to provide patient financing. You can apply online at CareCredit or ask us about CareCredit at your next visit. Additionally, our practice offers an in-house financing option for orthodontics. If you or your child are in need of orthodontic treatment, make sure to talk with us about our payment plan.
